UnitedHealthOne offers a variety of plan options to choose and can be easily customized to fit most budgets. The two plans that we recommend is their Copay Select for a traditional type plan and the HSA100 for their HSA Plan.
UnitedHealthOne Copay Select
Some of the highlights of the Copay select are as follows:
- $35 copayment for Primary Care Physician and Specialist (history and exam)
- Deductible options of $500, $1,000, $1,500, $2,500, $5,000, $7,500, or $10,000
- Maximum 2 deductibles per family
- Coinsurance options of 0% ($0), 20% ($3,000) or 30% ($5,000)
- $35 copayment for preventative visit. You then pay chosen coinsurance for X-ray and lab, child immunizations, pap smear, mammograms, and PSA screenings
- $15 copayment for generics not subject to drug deductible.
- Brand name drugs subject to $200 deductible. Maximum benefit for brand drugs is $3,000. However an unlimited benefit is available for additional cost and we highly recommend upgrading this feature.
- Comes with FACT membership which give you discounts for Vision and Dental discounts and some other unique discounts.
- $3,000,000 lifetime maximum benefit. Upgradeable to $5,000,000 (recommended)
UnitedHealthOne HSA100
Some of the highlights of the HSA100 are:
- Individual deductible options of $1250/$2500/$3000/$3500/$5000
- Family deductible options of $2500/$5000/$6000/$7000/$10000
- 100% coverage once the individual or family deductible is satisfied
- $35 copayment for preventative visit. X-ray, child immunizations, pap smear, mammograms, and PSA screenings covered 100%
- Prescription drugs covered 100% once individual or family deductible is met. $3000 annual maximum per person per calander year. No annual maximum plan enhancement available and highly recomended.
- $3,000,000 lifetime maximum benefit. Upgradeable to $5,000,000 (recommended)
